WEL’s New Model for Delivering Senior Housing and Health Care: Senior Health & Housing Initiative for Transformation (SHIFT)

WEL is in the process of designing a concept that we expect could significantly improve the delivery of senior housing and health care services. The SHIFT model is a new concept that transforms the funding and delivery of aging services for middle-income Americans. SHIFT is a comprehensive, integrated and holistic approach to housing and health care for the senior population – addressing their physical, emotional and lifestyle needs – that enhances access and improves outcomes at a lower overall cost.

We believe SHIFT is a model that could potentially be adapted to a much broader
constituency. SHIFT would funnel federal, state and private funding streams through a capitated continuing care retirement community (CCRC) model to meet and pay for the needs of seniors now falling through the cracks.

We suggest that SHIFT results in:
  • Improved access for the vast population of seniors who are not at the upper end of the economic spectrum
  • Provider flexibility and incentive to spend funds in a manner that reduces overall cost to the system by delivering services in an optimal manner and achieving superior outcomes
  • More effective chronic disease management and/or a deferral of the onset of those chronic and fatal disease states through effective prevention programs
  • The ability to address the challenges of isolation, nutrition and depression amongst elders
  • A possible expansion of Medicaid funding beyond skilled nursing, simultaneously reducing overall Medicaid spending
  • A reduction in the costs of providing health care for those being served under this model. These expenditures would be less than payments for an equivalent group under a fee-for-service Medicare program

We are proposing to test these beliefs through a pilot program. Once complete,
with our research and results published and presented to governmental entities,
we believe the outcome will be a paradigm shift in the way funding streams for
aging services are directed.
